Why Complete Records Matter

A comprehensive medical record review is the foundation of CIREN's eligibility determination. Without accurate, complete records, the medical team cannot determine whether the patient's condition is appropriate for intensive rehabilitation, whether the patient is medically stable for travel, or what treatment plan would be most beneficial.

Submitting complete records upfront ensures that the medical team can provide an accurate eligibility determination within 5–10 business days. Incomplete records may delay the review or result in an inaccurate assessment.

This guide explains what records are needed, how to prepare them, and how to submit them.

Required Medical Records

The following records are required for a comprehensive neurorehabilitation evaluation:

1. Neurological Assessment — A recent assessment by a neurologist describing the diagnosis, condition severity, and current status. This should include the date of diagnosis, type of condition (e.g., ischemic stroke, TBI, SCI level), and current symptoms.

2. Imaging Studies — Brain MRI and/or spinal MRI (depending on the condition) on CD or via secure digital transfer. Images should be recent (within 12 months) and include the radiologist's report. CT scans can be submitted if MRI is not available.

3. Blood Work — Recent laboratory results (within 6 months) including: complete blood count (CBC), comprehensive metabolic panel (CMP), thyroid function, vitamin B12, folate, fasting glucose, HbA1c, lipid panel, and any condition-specific tests.

4. Current Medication List — A complete list of all prescription medications, over-the-counter drugs, and supplements, with dosages and frequencies.

5. Medical History — A summary of the condition's onset, progression, prior treatments and responses, and any other relevant medical conditions.

6. Rehabilitation History — Records of any prior rehabilitation, including therapy notes, functional assessments, and outcomes.

7. Physician's Note — A letter from the patient's current neurologist or physician summarizing the current diagnosis, treatment plan, and recommendation for intensive rehabilitation.

How to Submit Records

Medical records can be submitted through SMC Salud's secure online portal:

1. Contact SMC Salud's international patient department to initiate the process. 2. Upload each document as a separate file (PDF, JPG, or PNG format). 3. For imaging studies (MRI/CT), upload the CD contents or provide a secure download link. 4. Include English or Spanish translations for documents in other languages (SMC Salud can arrange translation if needed). 5. Provide contact information for the patient's current neurologist for coordination.

The submission is secure and confidential. All records are reviewed only by CIREN's medical team and are not shared with third parties.

What Happens After Submission

After records are submitted:

1. Initial Review (1–3 business days) — SMC Salud's coordination team verifies that all required records have been received and are complete. 2. Medical Team Review (3–7 business days) — CIREN's medical team reviews the records, assesses the condition, and determines eligibility. 3. Treatment Proposal (within 5–10 business days total) — If eligible, the patient receives a personalized treatment proposal including program duration, components, and cost. 4. Questions & Coordination — SMC Salud's coordinators answer questions about the proposal and coordinate next steps (travel, accommodation, visa).

The review is complimentary — there is no obligation to travel after receiving the eligibility determination. If the patient is not eligible, the team provides recommendations for alternative approaches.

Frequently Asked Questions

What if I don't have recent imaging studies?

If your imaging studies are more than 12 months old, the medical team may request updated images before making an eligibility determination. If you cannot obtain recent imaging before submission, submit what you have and note the dates. CIREN can arrange imaging on arrival if needed, but having recent imaging before submission helps the team provide a more accurate assessment.

Can records be in a language other than English or Spanish?

Yes. SMC Salud can arrange professional medical translation of records from most languages. Submit the records in their original language and note the language. The coordination team will arrange translation as needed at no additional charge.

How secure is the record submission process?

Records are submitted through SMC Salud's secure online portal. All records are confidential and are reviewed only by CIREN's medical team. They are not shared with third parties. The submission process complies with applicable data protection standards.
