Understanding Eligibility

Cuba's lung cancer immunotherapies — CIMAvax-EGF and Vaxira (racotumomab) — are not appropriate for every lung cancer patient. These are therapeutic vaccines designed for a specific subset of patients, and eligibility is carefully determined through a comprehensive medical review by Cuba's oncology team.

Understanding the eligibility criteria before beginning the consultation process helps patients and families prepare the right medical documentation and set realistic expectations about the treatment pathway.

Cancer Type & Stage Requirements

CIMAvax-EGF and Vaxira are indicated for:

• Non-small cell lung cancer (NSCLC) — This includes adenocarcinoma, squamous cell carcinoma, and large cell carcinoma. NSCLC accounts for approximately 85% of all lung cancer cases. • Advanced stage (Stage IIIB or IV) — The vaccines are designed for patients with advanced disease where curative surgery is not an option, and who have completed or are completing first-line systemic treatment. • Following first-line therapy — Candidates must have completed or be completing first-line treatment (platinum-based chemotherapy, radiation, or a combination). The vaccines are used as maintenance therapy, not as a first-line treatment.

Patients with small cell lung cancer (SCLC), early-stage NSCLC eligible for curative surgery, or cancers of other primary sites are generally not candidates for these specific immunotherapies.

Medical Documentation Required

To determine eligibility, Cuba's oncology team requires a comprehensive medical dossier:

• Pathology report — Confirming the diagnosis of non-small cell lung cancer with histological subtype. • Staging documentation — CT, MRI, or PET scan reports showing the current extent of disease. • Tumor marker results — Including EGF blood levels if available (relevant for CIMAvax-EGF). • Treatment history — Complete record of all prior treatments including chemotherapy regimens, radiation plans, surgeries, and targeted therapies, with dates and response assessments. • Current imaging — Recent CT or PET scans (preferably within the last 4-6 weeks) showing current tumor status. • General health status — Recent blood work (complete blood count, liver and kidney function), performance status (ECOG score), and documentation of any coexisting conditions. • Current medication list — All medications and dosages.

SMC Salud's coordination team assists families in assembling and translating these documents for the Cuban medical review.

The Medical Review Process

The eligibility review follows a structured process:

1. Document submission — The patient or family submits all medical records through SMC Salud's secure consultation portal. 2. Initial review — SMC Salud's medical coordination team reviews the documents for completeness and translates key reports if needed. 3. Oncology team review — Cuba's oncology specialists at CIM and partner hospitals review the case, assessing cancer type, stage, prior treatment response, and overall health. 4. Eligibility determination — The team determines whether the patient meets criteria for CIMAvax-EGF, Vaxira, or CIMAher (for head and neck cancers). 5. Treatment proposal — If eligible, a personalized treatment proposal is issued with timeline, costs, and program details.

The initial review typically takes 24-48 hours after complete documentation is received. If additional information is needed, the coordination team will request specific records or tests.

Frequently Asked Questions

Can CIMAvax-EGF be used for early-stage lung cancer?

No. CIMAvax-EGF is indicated for advanced non-small cell lung cancer (Stage IIIB or IV) following completion of first-line treatment. Early-stage lung cancer that can be surgically removed is typically treated with surgery alone or with adjuvant therapy. The vaccine is not appropriate for early-stage, curable disease.

What if I haven't finished chemotherapy yet?

CIMAvax-EGF and Vaxira are used as maintenance therapy after first-line treatment is completed or nearly completed. If you are currently undergoing chemotherapy, Cuba's oncology team will review your treatment plan and advise whether to wait until first-line therapy is finished before beginning immunotherapy.

Does Cuba accept patients who have already been treated elsewhere?

Yes. In fact, most international patients seeking Cuba's lung cancer immunotherapy have already received first-line treatment in their home country. Cuba's oncology team reviews all prior treatment records to confirm that the patient is a suitable candidate for maintenance immunotherapy.
