Medical Records Needed for Alzheimer's Evaluation in Cuba
By the SMC Salud Medical Editorial Team
Why Medical Records Matter
A comprehensive medical record review is the foundation of Cuba's Alzheimer's evaluation process. Without accurate, complete records, the neurology team cannot determine cognitive stage, confirm diagnosis, or assess eligibility for NeuralCIM® therapy.
Submitting complete records upfront ensures that the medical team can provide an accurate treatment recommendation within 5–10 business days. Incomplete records may delay the review or result in an inaccurate eligibility assessment.
Required Medical Records
The following records are required for a comprehensive Alzheimer's evaluation review:
1. Neuropsychological Testing — Results of standardized cognitive tests (e.g., MMSE, MoCA, ADAS-Cog, or equivalent). These tests provide objective measures of memory, attention, language, and executive function. If testing has not been done, Cuba's team can arrange it during the in-clinic evaluation.
2. Brain MRI — Recent (within 12 months) brain MRI on CD or via secure digital transfer. The MRI should include axial, coronal, and sagittal views. If a CT scan is available instead, it can be submitted, but MRI is preferred for detailed brain assessment.
3. Blood Work — Recent (within 6 months) laboratory results including: complete blood count (CBC), comprehensive metabolic panel (CMP), thyroid function (TSH, T3, T4), vitamin B12, folate, fasting glucose, HbA1c, lipid panel, and any other relevant tests.
4. Current Medication List — A complete list of all current medications, dosages, and frequencies. Include prescription medications, over-the-counter drugs, and supplements.
5. Medical History — A summary of the patient's medical history, including: date of first symptoms, symptom progression, previous diagnoses, prior treatments and responses, family medical history (especially dementia history), and any other relevant medical conditions.
6. Physician's Note — A note or letter from the patient's current neurologist or primary care physician, if available, summarizing the current diagnosis and treatment plan.
How to Submit Records
Medical records can be submitted through SMC Salud's secure online portal:
1. Create an account or log in to the patient portal. 2. Upload each document as a separate file (PDF, JPG, or PNG format). 3. For imaging studies (MRI/CT), upload the CD contents or provide a secure download link. 4. Include English translations for documents in other languages (SMC Salud can arrange translation if needed). 5. Provide contact information for the patient's current neurologist for coordination.
The submission is secure and HIPAA-compliant. All records are reviewed only by Cuba's medical team and are not shared with third parties.
What Happens After Submission
After records are submitted:
1. Initial Review (1–3 business days) — SMC Salud's coordination team verifies that all required records have been received and are complete. 2. Medical Team Review (3–7 business days) — Cuba's neurology team reviews the records, assesses cognitive stage, and determines eligibility for NeuralCIM® therapy. 3. Treatment Proposal (within 5–10 business days total) — Families receive a personalized treatment proposal including: eligibility determination, recommended program duration, treatment components, estimated cost, and proposed start date. 4. Questions & Coordination — SMC Salud's coordinators are available to answer questions about the proposal and coordinate next steps.
The review is complimentary — there is no obligation to travel. If the patient is not eligible for NeuralCIM®, the team provides recommendations for alternative approaches or confirms that current domestic care is appropriate.
Frequently Asked Questions
What if I don't have neuropsychological testing results?
If neuropsychological testing has not been done, Cuba's team can arrange it during the in-clinic evaluation in Havana. However, having testing results before submission helps the medical team provide a more accurate eligibility assessment. If you don't have testing, submit the other records and note that testing will be done in Cuba.
How recent do the MRI and blood work need to be?
Brain MRI should be within the last 12 months. Blood work should be within the last 6 months. If your records are older, the medical team may request updated tests before making an eligibility determination. If you cannot obtain recent tests before submission, submit what you have and note the dates.
Can records be in a language other than English?
Yes. SMC Salud can arrange translation of medical records from most languages. Submit the records in their original language and note the language. The coordination team will arrange professional medical translation as needed. There is no additional charge for translation.
